You just need to notify all the important agencies (e.g., SS office, Passport, etc.) -- here's a free website that provides links to all of these -- http://newlastname.org. You'll get charged at the DMV, and maybe your Passport. The SS card is free.

Your child's name will need to be changed on its own.
